Reunion Registration Information

Please complete the Registration form found here and in the PDA Newsletter. You may pay registration fees by check or by credit card. Instructions for paying by check are on the form.

To pay by credit card, please print and complete the Registration Form. Then return to this page and go to our Pay by Credit Card page for instructions. Credit card charges are done using Pay Pal.

If you are unable to print the registration form and do not have a PDA Newsletter with the form, please contact the reunion host to have one mailed to you.

Cancellation Policy: $10.00 cancellation fee, no refunds issued after May 12, 2008.
