Instructions for Paying by Credit or Debit Card

1. Registration Form:

You must print, complete and mail your registration form by the same deadline as payments. On the registration form, write or check the box "Pay Online". The name of the person paying via PayPal must be the first name on the form so we can match your form with your payment.

  • Print a Registration Form (this is a PDF with form fields so you can save to your computer and type your information before printing, or just print and complete by hand).
  • The form is also available in the January and April issues of the PDA Newsletter.

3. Credit/Debit Card Payment Rules and Conditions:

Use the form below to pay using PayPal. The PDA uses PayPal for credit card transactions. The PDA does not store your credit card information on our web site. PayPal is a leading credit and debit card transaction processing company providing PDA with the ability to accept credit card payments at a reasonable cost.

The following apply to all card payments:

  1. Payments accepted for the all-inclusive Package only.
  2. All credit card and financial transactions are performed on PayPal's secured web site.
  3. All charges are in US Dollars.
  4. All prices are "per person". Be sure to indicate the number of persons in the "Quantity" box for each item you are paying for.
  5. A 3% handling fee will be added to your balance for credit card services (shown as a "shipping and handling fee"). This is the service fee charged by PayPal for processing your credit card transactions.
  6. With each registration deadline, the fees increase, so save by registering early.
  7. If you wish to register after May 30, please contact the Reunion Host for instructions. You may register on-site, but we will appreciate any counts for the banquet you can give us three weeks prior to your arrival.
  8. Spouse, partner and dependant children in your household are considered members if you're dues are currently paid in full.
  9. The PDA, it's members, and officers, are not responsible for your loss of property or personal injury. Attendance at the reunion is at your own risk.
  10. Cancellation Policy: $10.00 cancellation fee, no refunds issued after May 22, 2011.

All Inclusive Package Registration Fee:
includes reunion registration and Saturday's banquet.

PDA Member: $107.00 + $3.21 (3% fee) each
= $110.21
Non-Member: $117.00 + $3.51 (3% fee) each
